The Area of Greatest Need Fund exists to provide flexible charitable resources that enable The Foundation at Mountain Pacific to address the most pressing needs, emerging opportunities, and strategic priorities that advance its mission. Contributions to this fund may be used wherever the Foundation determines they will have the greatest impact.
Medication Access, Safety and Chronic Condition Support
The Medication Access, Safety and Chronic Condition Support Fund advances safe, effective medication management and chronic condition support to improve health outcomes, reduce risk, and lower avoidable healthcare costs. Building on Mountain Pacific’s expertise, the Fund supports innovative, evidence-informed approaches, including pharmacist-led care coordination that integrates clinical pharmacy expertise into the healthcare team. This work improves medication safety and adherence, helps individuals manage complex medications and chronic conditions, and reduces preventable complications, adverse drug events, hospital readmissions, and other avoidable healthcare use.
Access to Care and Essential Supports
The Access to Care and Essential Supports Fund reduces non-clinical barriers that prevent individuals from accessing and following through on needed healthcare. The Fund focuses on innovative transportation solutions that improve access to care, particularly for vulnerable populations and individuals in rural and underserved communities.
Whole-Person Behavioral Health and Community Well-Being
The Whole-Person Behavioral Health and Community Well-Being Fund advances community-based behavioral health solutions that improve access to care that is already available, strengthen continuity of care, and support long-term recovery. The Fund focuses on addressing gaps between successfully transitioning from crisis intervention to ongoing care and recovery supports.
